Park Place Health & Dental Clinic Services Provided:







The Clinic provides low cost services in dental extractions (teeth pulled), restorations (fillings) and hygiene (cleanings) for those who qualify.







Additionally, they do offer Partials ($100 per partial) and Dentures ($200 per plate) at an additional cost.

To Qualify:







Proof of Norfolk Residency







First, you have to be a Norfolk resident to be seen at this Clinic. You need to bring Proof of Norfolk Residency.







Forms for Residency Include: 







    1. A current lease with your name and address that shows current and valid dates







    2. Rent statement within 30 days with your name and address







    3. A piece of mail that is not junk with your name and address on it within 30 days (post marked on dated on the mail)







    4. If a resident of a local shelter program, then a letter from the program on their letter head stating the time and date you have been staying with            them and for how long.Proof of Household Income







Second, you will need to bring proof of  total household income.             Their clinic serves the homeless, low-income and uninsured of the city of Norfolk.







            Because of this you will need to provide documentation that shows total household to see if you qualify.







            Forms of Income Identification required are:







2 most recent pay stubs if paid by-weekly or 4 most recent pay stubs if paid weeklyMost current SSI or Disability award letter for the current yearSNAP Benefit award letter still validIf no income, than a W-6 from the employment commission on Virginia Beach Blvd            They need all the income of the individuals claimed on your taxes in order to qualify your income for services

Third, you need to bring a valid picture ID.







This could easily be a current, non-expired Driver's License or a valid ID card. For New Patients:







The $25 fee is due when submitting the application.







That fee covers your first appointment cost.







Any other follow up appointments needed are then $25 each after that.







Eligibility Screenings:







They use an Eligibility Screening system that will take no more than 15 - 20 minutes once the process is started and as long as they have an appointment available.







Screenings are in person Monday thru Thursday between 9 am to 2:30 pm with a clinic representative who will go over the forms you have provided (see list of required items below).







It is in the best interest to call ahead of time to see if they have something open.







Important:







Their clinic only sees those who have no dental insurance of any kind.







You must also be a Norfolk resident to qualify for their services.







All of their appointments are $25.







It depends on each individuals severity on whether how much treatment gets completed at each visit.







PPH&DC accepts only cash or money orders.







If you are a first time patient or if you are re-certifying your paperwork, you will need to pay the $25 up front prior to your first appointment.
