Obtaining an Application Comprehensive Care Program Applications for this program are available every month on a first-come first-serve basis.

There are a limited number of applications given out each month, so they encourage their potential patients to arrive early in order to ensure the best possibility of obtaining an application.

Please call one of their Patient Care Coordinators at 615-329-4790 (Nashville office) or 615-225-4141 (Murfreesboro office) to find out when they are having their next Application Day.

Download their What to Bring PDF for a full list of the required documents necessary to complete your application.

If you receive an application, their Patient Care Coordinators will provide a brief orientation on the program and how to correctly fill out the application.

They will also remain available throughout the day to answer any questions you may have.

They are here to help! Emergency Program

If you believe you have an emergency dental need and have not already called their office, please do so as we do not accept walk-ins.

They have emergency appointments available each day on a first-come, first-served basis.

When you arrive for your emergency appointment, you will fill out a basic registration form and be seen by one of their doctors.

Comprehensive New Patient Appointment

They want your first new patient appointment with them to be a great start to your smile journey.

Here is what to expect at your first visit:

  • Greeting by their team members
  • They are excited that you have chosen to make your dental health a priority, and they are happy to be a part of your tooth transformation.
  • Review of medical and dental history and discussion of concerns and goals
  • They want to hear your story! How has your dental health affected your family, employment, and self-esteem? What do you want your smile to represent after completing our program? They will listen to your concerns so they are able to personalize your treatment to fit their mutual goals.
  • X-rays and treatment plan
  • All of their comprehensive patients receive full diagnostic x-rays that are reviewed by their doctors.
  • They will then complete a comprehensive intra-oral and extra-oral exam, as well as an oral cancer screening.
  • After reviewing your dental needs with you, they will design and complete a treatment plan based upon our mutual restorative goals in order to ensure a successful, healthy smile and program completion.

Appointment length for their new patients:

Comprehensive Exam and Phase 1 Plan (fillings, cleanings, extractions): 1.5 hours
Comprehensive Exam and Phase 2 Plan (crowns, root canals, partials): 2 hours
