The mission of HealthLink Medical Center (HLMC) is to address the primary medical and dental healthcare needs of working uninsured individuals who meet their eligibility requirements. HealthLink Medical Center is dedicated to providing all patient services in a thorough, cost-effective, and respectful manner, and is committed to continual re-evaluation and improvement of its care-delivery practice.















HealthLink provides basic dental services for eligible patients,







including exams, X-rays, fillings, cleanings and simple extractions.















Eligibility requirements must be met to receive free dental care at HealthLink.















HealthLink patients must:







Be between 18 and 65 years of age.







Live in either Bucks or Montgomery Counties.







Be working and have proof of employment.







Earn within 250% of the Federal Poverty Guidelines. (See table to right)















Eligibility will be reviewed at the first visit and annually in the Spring. Patients must continue to show proof of meeting all eligibility requirements each year to continue receiving care.























At your eligibility appointment, you will be required to bring with you:







Most recent 2 months of pay stubs;







Completed Federal tax return (Form 1040) & schedules, if applicable, for the current year;







Proof of residency, such as a utility bill or signed rental lease; and







Photo ID (driver's license, passport, or work ID).
