Ko'olauloa Community Health and Wellness Center (KCHWC) is a private, non-profit, community-based New Start Federally Qualified Health Center in the Ko'olauloa district of Kahuku, Hawaii. Services include acute care, chronic care, physical exams, immunizations, family planning, minor surgery, mental health counseling, podiatry, Native Hawaiian Healing, home visits, health education, and eligibility/outreach services. Oral health, nutrition, substance abuse, and case management services will be offered in the near future. Priority populations include the poor, uninsured, underinsured, homeless, Native Hawaiian, Pacific Islanders, immigrants, agricultural workers, youth, and the elderly.
