Cornell Scott-Hill Health Center is a federally qualified community health center established in 1968 in a collaboration between the community and Yale School of Medicine.







The first community health center in Connecticut, the Cornell Scott-Hill Health Center has a long history of serving New Haven neighborhoods, which are among the poorest in the State.







Cornell Scott-Hill Health Center also provides health care services to those from the City of West Haven and towns in the Lower Naugatuck Valley, Ansonia, Derby, Seymour, Shelton, Naugatuck and Oxford.







Sliding Fee ScaleThe Cornell Scott-Hill Health Center seeks to make its services more affordable for uninsured, low-income patients.







If you are uninsured and a low-income patient, you may qualify for reduced fees based on your income.







This is called a sliding-fee scale.







You may apply for one of five discount levels, based on annual income and family size.







The discount off of our standard charges will remain valid for one year after the date of application, unless you qualify for or secure insurance coverage in the interim.







They will notify you, in writing, if you qualify.







Thereafter, you are expected to report any change in your income, family size or insurance status.
