Provides a nonprofit community health center that offers primary medical care, dental services and behavioral health care to individuals and families experiencing homelessness, and residents of public housing.

Residents of Cuyahoga County who are age 3 or older, and uninsured or underinsured. Targets those who are homeless or living in public housing.

Hours M, T, TH : 8:30am-4:30pm; W: 8:30am-6:00pm; F: 8:30am-3:00pm.
Fees: Sliding scale (slides to zero, except for dentures or partials, which will always require a minimum fee); Third-party payment accepted: Medicare, Medicaid and private insurance. Uninsured clients pay a fixed fee for the initial consultation.
